Exam and Glasses Assistance Program

Service to the blind and visually impaired is one of the Lions club’s most significant

activities. Montrose Lions Club has developed this program to financially assist those in

this community who need an eye exam and glasses. Montrose Lions club is a non-profit

organization that uses limited funding for this project and is looking for applicants who

are in genuine need.

Requirements and conditions for application:

- Applicants must live within our service area which includes, Montrose County, San Miguel County, Ouray County, Hinsdale County, and the western portion of Gunnison County.

- Applicants must provide an appropriate method of contact so they can be reached about the status of aid. If this is someone other than the applicant or parent/guardian, a liaison must be named, and a phone number provided.

• Although a Spanish version of this document will be provided, Montrose Lions do not have a Spanish speaking member. Applicants are asked to provide an English-speaking liaison.

• Applicants may reapply for aid after one year for children 17 and younger, and every 18 months for adults 18 and over.

• Applicants who have been selected are approved up to and not exceeding $112 on a vision exam and up to and not exceeding $45 for for single vision and $65 for bi-focal glasses. These amounts do not have a cash back value to the recipient.

• Applicants will be asked to receive services through select providers, so that the aid amount funds the entire cost of the eye exam and glasses.

• Glasses provided through this service are basic, with no frills. The recipient of aid from this program is not permitted to purchase out of pocket add-ons for glasses as this indicates that he or she is not in genuine need.
